A short survey that captures what virtual care already happens in your home, what works well, and where the gaps are.  It's yours to own and can be copied, adapted and sent to your managers.

Facility managers, clinical care managers and care staff who help run or plan virtual care.

Why it helps

It gives you a clear picture of where your home is now. You use it to restart equipment that is sitting idle, tighten the way virtual care fits your workflows, and decide whether your home is a good fit for a new model of care.
